Instructions for obtaining the necessary documents

Medical Report from Health Portal

1. Log in to the terviseportaal.ee with Mobiil-ID, Smart ID or ID-Card.
2. Click on the left ’’My Health History’’. Find the documents related to the case - Receptions / Analysis / Observations / Ambulance / Hospital treatment / Surgeries.
3. For the chosen cases click left ’’View case’’or ’’View result’’.
4. In the last section ’’General’’ click on the left link ’’Show more’’ and ’’View full document’’.
5. On the left you can find ’’Print’’ – save it as PDF.
6. Send the PDF to to your Insurance claim handler.

Sick leave certificate form Eesti.ee portal

1. Log into the portal with Mobiil-ID, Smart ID or ID-Card using THIS link.
2. Choose ’’Certificates of temporary incapacity for work’’ and click ’’Download PDF’’. 
3. Send that PDF to your Insurance claim handler

Income Information from eMTA

1. Log in with Mobiil-ID, Smart ID or ID-Card with THIS link.
2. Click on the blue link on the right ’’New certificate’’ and choose ’’Taxation data certificate’’. 
3. For the beginning and ending period choose 12 calendar months immediately preceding the occurrence of the insured event. The exact period is specified in the information letter sent after registration.
4. Make sure all the details are ticked and click on the green button ’’Compile certificate’’ on the right. 
5. Under the Copmiled sertificates click on the envelope on the right and send the sertificate in PDF format to your claims handler.
